Dietary approach to hyperphosphatemia in CKD patients

Abstract
There is good epidemiological evidence linking hyperphosphatemia and mortality among dialysis patients (1). Therefore, adequate control of hyperphosphatemia is a critical step in the routine clinical care of these patients. The main two factors determining serum phosphorus is a chronic dialysis patient are intestinal absorption and dialytic removal of phosphorus. In reality, however, the control of serum phosphorus levels in dialysis patients remains poor (2) reflecting the limited ability of current phosphorus binders to decrease intestinal phosphorus absorption as well as the inefficiency of the current 3-times-a-week hemodialysis or daily peritoneal dialysis to remove enough phosphorus in a patient ingesting an average American diet. Therefore, modification of dietary content of phosphorus is of the utmost importance in order to better control serum phosphorus and ultimately reduce the risk of cardiovascular morbidity and mortality in this population.Comments:
